Ambient AI medical scribe — sign in to continue
Uses invented patients. Athena, cloud sync, sending, and signing are off.
🛡️ Secure account sign-in. Your credentials verify your MLS account. Clinical workspace access is checked separately for this deployment and account. Do not enter patient information unless the workspace is enabled and your organization has approved its use.
Please answer what you can — your care team will review everything with you. When you're finished, press the green button at the bottom and hand the iPad back to the front desk.
The fastest way: take a photo of your insurance card and we'll fill this in for you.
For each section, tap the ONE statement that best describes you today. Skip any that don't apply. (Oswestry Disability Index)
Optional — feel free to skip these if this is your first visit.
Enter your OpenAI API key once to enable AI note generation.
🔑 Account & access
How this device connects to AI note generation, plus the name and specialty shown on your notes.
🔒 Your key stays on this computer; only sent to OpenAI. It is saved in this browser's local storage and never shared with anyone else.
✔ Key saved on this device.
🏥 Practice & provider
Your clinic and provider details. Saved to your MLS account so booking, patient messages, reports, and printed notes all use the same identity.
Optional. If blank, MLS searches Google Maps using your practice name and address.
Premium adds your logo to printed/PDF notes and white-labels them.
📝 Note defaults
Defaults applied to new notes. The note style is shared with MLS as a writing preference.
Tip: your standing follow-up interval lives below under Provider preferences.
🎚️ AI personalization
Standing instructions and defaults MLS applies to every note it writes for you.
Premium notes run on a priority lane for lower latency. GPT-5 mini is the newest and strongest for most notes, GPT-4o is a proven balanced choice, and GPT-4o mini is the quickest. Standard always uses GPT-4o mini.
Add reusable style/content rules and MLS follows them on every note. These are remembered instructions applied to the AI — not patient details.
e.g. Use APSO format · Keep HPI to 3 sentences · Always add return precautions · Never suggest benzodiazepines · Use my normal-exam template
Your standing comment is dropped into the Comment box on every new visit and woven into the generated note. Edit it per visit as needed.
e.g. Risk/benefit discussed; patient verbalized understanding. Follow up in 2 weeks or sooner if symptoms worsen.
🎨 Display
Appearance and workflow preferences for this device.
Left sidebar adds a collapse button; when collapsed, a slim icon rail stays on the left edge — always clickable, keyboard accessible, and it never covers your work.
When you pick a patient, show their chart beside the list instead of below it (wide screens).
✍️ Provider preferences
Saved credentials added to the bottom of every generated note under an "Electronically signed by:" line. Leave blank to append nothing.
e.g. Matthew W. Schaeffer, MD — Chester County Spine Care
Your standing follow-up window, shared with MLS as a preference for the plan/return-precautions.
e.g. 2 weeks
🧩 Features
Turn optional tools on or off for your practice. Off simply hides that tool — nothing else changes.
Computes total daily morphine-equivalents with CDC risk bands.
Auto-builds operative candidacy, conservative-care, and consent points on surgical visits.
Assesses whether a pasted course of care looks medically necessary and reasonable.
Estimates a patient's out-of-pocket cost from their entered insurance benefits.
Formats generated notes to match your imported template — manage templates under Menu → 📄 Templates.
🧩 Custom widgets — build your own auto-generated cards with the + Custom widget button in the header. They fill on every Generate, just like the built-in cards.
One question per line. These appear at the end of the 📋 Patient intake form (the header button you hand to the patient on the iPad), and the answers flow automatically into the patient's summary/history. Leave empty to hide the section.
🧰 App tabs
Keep MLS simple. The advanced tabs below are hidden until you switch them on, so a new doctor sees a clean app. Patients, Visit, History, Copilot (AI Studio), and Help are always available.
🔒 Security & privacy
Protect access on this device. Clearing saved data only affects this browser — it never touches server data.
Helps protect an open workspace when you step away. Resets on any activity; it is one control, not a compliance determination.
Removes this account's MLS settings, preferences, patients and notes saved in this browser. This cannot be undone. It does not delete anything on a server or affect other accounts/devices.
🔗 Integrations
Connections to outside systems like your EMR.
This is an optional, secondary schedule-read path. The clinician-started MLS Assist Today, Calendar, day, and month pulls remain available and are never replaced or started automatically.
Available from the hosted MLS serverOpen Settings or press Check status to read the server connection state. Nothing opens or connects by itself.
Client credentials and refresh tokens stay on the MLS server. The browser never stores an Athena client secret or connection claim.
MLS Assist now works automatically off your MLS login — no key needed, just stay signed in here. The Developer API key below is optional: only for the MLS FHIR API or advanced server use.
Drops finished notes into any web EMR, reads your Athena day for 📅 Pull schedule, and Autopilot can take steps for you with your approval.
chrome://extensions and press ↻ on MLS Assist.
chrome://extensions and turn on Developer mode (top-right).mls-assist-extension folder. (To update later, replace the folder and click ↻ Reload.)With MLS Assist installed, open a patient in your EMR (another tab), then click below — MLS reads that chart and pulls the patient + prior visits in here. No copy-paste.
API docs: scrivara-backend.onrender.com/fhir/docs
Move many patients and their visit history into MLS at once. Paste a JSON export below — shape: {"patients":[{"name":"Jane Doe","dob":"1980-05-01","mrn":"12345","history":[{"date":"2026-03-10","note":"..."}]}]}. Stored encrypted, same as in-app charts.
Schedule and chart-history pulls are clinician-started. Today/Calendar can read the day or month you request through MLS Assist while you are signed in to athenaOne. A supervised history import reads only the patient you selected. Neither action is a server-side nightly backup.
This nightly backup uses your EMR's official API connection (separate from the Visit tab's "Pull this day", which reads athenaOne through your office computer). How much it can back up depends on the access level your EMR granted — a patient-level connection backs up only that patient until your practice enables practice-wide API access.

You are executing the two-party BAA as the Business Associate. Signer: . Once countersigned, the executed record is stored server-side and both parties can download it.
Import a blank template/form (PDF or image). When enabled, MLS formats the generated note to match the template's structure and fields. Templates are stored locally on this device. Off by default — when off, generation is unchanged.
Select a template on the left to preview and edit it here.
Describe any card you want on the visit page. MLS can design dropdowns, checklists, status badges, scores and mini-charts into your widget — you approve the design, and from then on it appears with the other cards and auto-fills on every Generate — built only from what is documented, with [bracketed placeholders] for anything missing. Up to 10 widgets; they follow your account.
Ask where a button is or how to do something in MLS — answers are specific to this app, step by step.
Let's get your practice ready in a few quick steps — your details, schedule, Staff Prep, and key settings. Then a short guide to the core doctor workflow. You can change anything later in Settings.
Review the request, attach the correct patient chart if you treated the patient, and add or edit counsel's legal questions before generating. Nothing drafts until you press Generate report.
No patient history will be used unless you attach a patient chart above. The report is a DRAFT to verify, complete, and sign.
This report has blanks the AI couldn't fill from the records. Enter the information below and it will be inserted into the report — nothing is left in brackets. Leave a field empty to keep its bracket for now.
AI summarizes and extracts only what's in this document — it won't invent findings. Review before merging into the profile.
